Impatient for the end of his shift and exhausted from a hard day's work in front of a computer monitor, an unsuspecting office clerk decides to have a break and take part in a rather unusual online questionnaire. Eventually, as the man begins answering the unexpected and gradually startling queries, an ominous feeling that he is being watched closely starts to prevail, nevertheless, does this questionnaire really hold the answer to the mystery of the man's death? If he is honest and brave enough, he will surely find out.
